Community Health Workers (CHW)

MCD Public Health in collaboration with the Maine Migrant Health Program is working to develop training systems, a certification mechanism, awareness of, and support for community health workers in Maine. Both agencies have a history of training and employing Community Health Workers. Some elements of our current collaboration are funded through a contract with the State of Maine. MCD Public Health has a long history of promoting and supporting the training and deployment of community health workers to enhance the effectiveness of disease prevention interventions. In 2013 MCD Public Health partnered with the Maine Migrant Health Program and was awarded a contract with the Maine Department of Health and Human Services to support their effort to create a Community Health Worker pilot project and to craft standardized CHW training and certification. That scope of work is supported by a grant from the federal Centers for Medicare and Medicaid Services (CMS) to the State of Maine Department of Health and Human Services through an initiative identified as State Innovation Models (SIM). Documents and resources created to comply with the deliverables for that contract are identified herein through SIM branding and the application of the SIM / Maine DHHS style guide. Not all resources and certainly not all documents contained within this part of the website are associated with Maine DHHS' SIM initiative. This webpage was developed and is maintained with SIM and non-SIM resources.
